Full-Service Immunization Program

Freedom Pharmacy will research the immunization history of residents, utilizing NJIIS records, provider pharmacy vaccine reports, facility admission face sheets and eMAR records.  Our pharmacist will review records and reference the current CDC Immunization Schedule to determine the appropriate vaccine to be administered.  Insurance coverage will be investigated prior to the clinic.  

 

Freedom Pharmacy is a participating provider of New Jersey Immunization Information Systems (NJIIS) and will report all vaccines administered.  

 

Immunization clinics are provided on-site; no maximum number of patients is required.  

 

Vaccines are an integral part of an Antibiotic Stewardship program and Infection Prevention and Control.  The Influenza and pneumococcal vaccine must be offered to a resident residing in a nursing home, as per CMS.
